Consent to Personal Data Processing
Filling of web-forms when sending e-mail message about the quality of drugs on the e-mail address is info@derinat.ru means my consent to the collection and processing of personal data of "PharmPack" (hereinafter the "Company").
This action is carried out in accordance with Federal Law No. 152-FZ of July 27, 2006 "On Personal Data" in order to obtain information about adverse drug reactions. This Consent is given on the implementation of any action with regard to my personal data, which are necessary to achieve that goal.
The Consent include, is without not limited to: collection, systematization, accumulation, storage, clarification (updating, modification), use, distribution (including transfer), depersonalization, blocking, cross-border transfer of personal data, as well as any other actions with personal data taking into account the current legislation of the Russian Federation. In accordance with the requirements of the Federal Law No. 152-FZ "On Personal Data" of 27.07.2006, this Consent is equated to a written consent. The date of issue of this Consent is the date of sending the web-form / submission of an electronic message about a possible adverse drug reaction to the e-mail address of the Company info@derinat.ru.
This consent is valid indefinitely, but may be withdrawn by written application to the address: 127521, Moscow, Ann's Passage, premises 7, Building 1.
